Resident Caregiver
Provide 1-on-1 support and build meaningful bonds in a home-like setting.
Assist 1–2 residents with daily living activities (ADLs) including grooming, mobility, and personal care.
Prepare nutritious meals and maintain a clean, comfortable home environment.
Administer medications with precision and document care according to DHS 88 standards.
Drive and accompany residents to appointments and community outings (gym, zoo, parks, dentist’s and doctor’s visits).
Provide the emotional support and companionship that makes our house a home.

PRN Caregiver
Flexible shifts for those who want to provide high-quality care on an as-needed basis.
What You’ll Do:
Step in to provide consistent, high-quality care during staff vacations or as needs arise.
Maintain the resident’s routine exactly as they prefer it, ensuring no lapse in quality of life.
Provide a reliable presence that residents and families can count on.
Contracted RN
Ensure the highest standards of safety, clinical integrity, and regulatory excellence.
What You’ll Do:
Care Planning: Collaborate on the development and regular review of individualized care plans to ensure they meet the evolving health needs of our residents.
Nurse Delegation: Perform staff training and nurse delegation in accordance with Wisconsin DHS 88 regulations, particularly regarding medication administration and specialized tasks.
Health Monitoring: Provide periodic clinical reviews of resident health status, monitoring for changes in condition and coordinating with primary physicians and MCO case managers.
Quality Assurance: Audit medication logs and health documentation to ensure absolute accuracy and compliance with state standards.
Who You Are:
A Registered Nurse (RN) currently licensed in the state of Wisconsin.
Knowledgeable in DHS 88 regulations or experienced in community-based residential care.
A detail-oriented professional who values clear communication and person-centered outcomes.
