Resident Caregiver

Provide 1-on-1 support and build meaningful bonds in a home-like setting.

Assist 1–2 residents with daily living activities (ADLs) including grooming, mobility, and personal care.

  • Prepare nutritious meals and maintain a clean, comfortable home environment.

  • Administer medications with precision and document care according to DHS 88 standards.

  • Drive and accompany residents to appointments and community outings (gym, zoo, parks, dentist’s and doctor’s visits).

  • Provide the emotional support and companionship that makes our house a home.

PRN Caregiver

Flexible shifts for those who want to provide high-quality care on an as-needed basis.

What You’ll Do:

  • Step in to provide consistent, high-quality care during staff vacations or as needs arise.

  • Maintain the resident’s routine exactly as they prefer it, ensuring no lapse in quality of life.

  • Provide a reliable presence that residents and families can count on.

Contracted RN

Ensure the highest standards of safety, clinical integrity, and regulatory excellence.

What You’ll Do:

  • Care Planning: Collaborate on the development and regular review of individualized care plans to ensure they meet the evolving health needs of our residents.

  • Nurse Delegation: Perform staff training and nurse delegation in accordance with Wisconsin DHS 88 regulations, particularly regarding medication administration and specialized tasks.

  • Health Monitoring: Provide periodic clinical reviews of resident health status, monitoring for changes in condition and coordinating with primary physicians and MCO case managers.

  • Quality Assurance: Audit medication logs and health documentation to ensure absolute accuracy and compliance with state standards.

Who You Are:

  • A Registered Nurse (RN) currently licensed in the state of Wisconsin.

  • Knowledgeable in DHS 88 regulations or experienced in community-based residential care.

  • A detail-oriented professional who values clear communication and person-centered outcomes.
